What do I need to bring to my child’s first visit?

Please bring: Your child’s insurance card, Photo ID for the parent/guardian, Immunization records, Any prior medical records, list of any current medication

What if I am running late?

If you’re running late, please give our office a call and we will always try our best to accommodate you.

Can I bring a stroller or car seat into the clinic?

Yes. We do our best to accommodate families with infants and young children.

Can I get a school note or clearance?

Yes, we provide school, camp, and sports clearance forms at the time of visit.

Can I decline student involvement?

Absolutely. Your comfort is our priority. If you prefer that no students participate in the visit, just let our staff know, your care will not be affected in any way.

Are vaccinations required?

We follow the CDC-recommended immunization schedule. Required vaccines depend on age and school/daycare requirements. We also work on modified schedules

Do you treat asthma, allergies, and chronic conditions?

Yes. We diagnose and manage common pediatric chronic conditions such as asthma, eczema, allergies, obesity, and ADHD.

How can I access my child’s medical records?

You can request medical records through our front desk or by calling and/or emailing our office. We provide printed or electronic copies depending on your needs.

Do you provide referrals to specialists if needed?

We provide patients with referrals to specialists as needed.

Will you provide guidance on nutrition, safety, behavior, mental health, preventive care, and parenting support?

We provide our patients with different resources and information through emails, texts, and flyers to best accommodate families.
