What is the USCIS immigration medical exam (Form I-693)?
It is a required medical examination for people applying for U.S. permanent residency (Green Card). Only a USCIS-designated Civil Surgeon can complete the exam and fill out Form I-693.
Who can perform the immigration physical?
Only a licensed Civil Surgeon approved by USCIS — our clinic provides official I-693 examinations and sealed forms. Dr. George Vermenton has been approved by USCIS to perform immigration physicals.
How many visits are required for an immigration physical?
Most patients require two visits:
First visit: physical exam + labs + vaccine review. Second visit: review results + completion and sealing of the I-693 form.
Do you perform the TB test at your clinic?
Yes. We use the USCIS-approved Quantiferon TB blood test
Do you accept insurance for the immigration exam?
Immigration physicals are a non-covered service, so they must be paid out-of-pocket. Some insurances may cover vaccines and or labs — ask us for details.
How long is the sealed I-693 valid for?
Once signed by the Civil Surgeon, the exam is valid for 2 years, per USCIS rules.
What if I don’t have vaccination records?
No problem. We can perform a blood test (titers) to check immunity or administer required vaccines during your visit.
Do I need to fast before the immigration physical exam?
No fasting is required.
What tests are required for the I-693 exam?
USCIS requires the following, depending on age: Tuberculosis screening (TB blood test / Quantiferon), Syphilis blood test (ages 15+), Gonorrhea urine test (ages 15+), and Vaccination verification (MMR, Varicella, Tdap, Flu, Hep B, etc.)
Can I get all the required vaccines at your clinic?
Yes. We provide all USCIS-required vaccines or can refer you to a nearby pharmacy, depending on availability.
